Is Salbutamol Pump Recommended In Case Of Extreme Tiredness And Shortness Of Breath?

Hello I am 42 years old and has recently been put onto a salbutamol pump due to having extreme tiredness and shortness of breath. I also suffer from hayfever which started off by being seasonal but I appear to have symptoms the entire year round. My problem is that I have also developed ectopic heartbeats which are making me anxious as I feel they induce a cough which further affects my breathing.
I need help in understanding why these symptoms are occurring and whether I am doing anything that induces them. Any advice would be helpful. Thanks

Hello,

Thank you for asking at HCM.

I went through your history and would like to make suggestions for you as follows:

1. As you have shortness of breath, hay fever, initially seasonal progressing to perennial, all these symptoms suggest possibility of asthma, may be allergic.

2. For confirmation of diagnosis of asthma a test known as Spirometry with post-bronchodilator reversibility is necessary.

3. At present, based on your history, I would suggest you allergy testing for common air-born allergens such as house dust mite, insect proteins, molds, pollens, animal dander, etc. This will help you identify the substance causing allergies to you as well as to know the measures to avoid them.

Based on allergy testing, an Allergist-Immunologist also may prescribe you allergen specific immunotherapy which works on immune system to gradually improve allergy symptoms over a long time period.

4. As you have symptoms year around, I usually suggest my such patients regular montelukast + levocetirizine and a preventer inhaler such as inhaled corticosteroids. These should to be taken regularly under physician's guidance to avoid symptoms of asthma.

5. For patients with ectopic heartbeats or other heart problems, I usually suggest them to use levosalbutamol in place of salbutamol as levosalbutamol is more safe in terms of cardiac symptoms. Also, regular use of inhaled corticosteroids will reduce your requirement of salbutamol/levosalbutamol.

Please note that both salbutamol and levosalbutamol are "rescue" type of inhalers which are to be used on as-and-when-needed basis. Inhaled corticosteroids are "preventer" type of inhalers which are to be taken on regular basis.

6. Regular breathing exercises will help you in a long run to improve your lung capacity.

7. A healthy diet rich in vitamins, minerals and antioxidants will also help gradually to improve your immune system. Please note that allergy is a disorder of immune system. Such diet with adequate fluids during day will also improve your symptoms of tiredness.

8. In general, please avoid exposure to dusts, smokes and air pollution as much as possible.
