Is Self Dilate After Meatal Urethral Dilation Surgery Required?

Hi, I had a Meatal Urethral Dilatation recently. I was advised by my consultant to have this procedure due to tightening of the meatal opening. A condition that started 2 years ago and steadily got worse. The consultant said it had an 80% success rate. He mentioned I would be taught to self dilatate after the operation. I was shown this procedure today and find this straight forward to do. However, whether its due to nerves or concentrating on the information given, I always have a good few questions I d wished I d asked at the time. I am also finding it difficult to contact my consultant. Therefore any help on the following questions would be gratefully received. 1), Exactly how long will I be expected to self dilatate? 2), Was the procedure I had done considered a success providing I self dilatate forever more or occasionally? 4), I have read on the internet a Meatatomy provides a much better chance of a complete cure. Is this true? 5), Does a Meatatomy still involve self dilatation after the operation, if so, for how long? 6), If I was to follow my consultants advice to self dilatate every day, I have read on the internet, in the long term causes secondary healing and scarring and causes a tighter stricture. Is this true? 7), Please could you provide any advice on any other treatments to provide a long term cure and exactly what they may entail? Many thanks for any answers you can provide here, it is very much appreciated. Stuart

General & Family Physician 's  Response
Hello,

1. Self-dilatation usually requires a lifelong period.
2. The procedure you had undergone is primary relief of urethral stricture or narrowing, not curative.
3. Meatotomy is only recommended when there is narrow meatus and as the metal dilator has passed your urethra, means that you have not narrow meatus and you are not a candidate of meatotomy.
4. Meatotomy does not require dilatation if there is no associated urethral narrowing.
5. Even though you follow all directions of your consultant, there are remote chances of scarring or fibrosis of the urethra in future though it would be very less as compared to the instance in which you are not following any advise.
6. Other best treatment option is urethroplasty which is curative for a lifetime.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Purva Patel, General & Family Physician
