Is Surgery Needed For Severe Pain In Lower Part Of The Body Post An Injury?

Hi there, I had a small horseback riding fall yesterday. My horse refused a jump and I fell over his neck and landed (left side) head first then hip to the ground. I was fine yesterday but this morning I could barely walk. Abdomen is very distended, tender and I have a lot of pain from the lower flank down to the groin area. Touching it, coughing, lifting or sitting up really hurts. What are the odds of this being a hernia? (No nausea, vomitting fever for now) Or could this be simply a tear in the obliques? If it s torn, does it require surgery? Thank you
