I have sudden and severe boring epigastric and pain in the upper right hypochondrium, I did ultrasonography on the abdomen which revealed enlarged liver and displays diffuse bright and mildly coarse echopattern. No focal lesions or dilated biliary radicles. Portal vein is normal in size. CBD is dilated. The gall bladder was excised. No masses or definite stones could be seen. Spleen and pancreas are free. No enlarged para-aortic LNS. No ascites. I did some laboratory tests and the results are: ALT and AST are normal, serum amylase and lipase are within normal range, CBC is normal. CRP is elevated to 21.7 and the ESR ia also elevated to 30 What is your provisional diagnosis of my case? and what is the treatment?
I would like to know your cholesterol numbers and then proceed with making a diagnosis. It might be possible that you are having gastric ulcers or geastroesophageal reflux which can cause significant pain in the region. As your gall bladder is excised and you do not have jaundice or biliary obstruction, a cause relating to biliary tract obstruction is ruled out. A coarse liver may need liver biopsy to know the exact nature of liver cell injury if any.
