General & Family Physician 's  Response
Hello
I share your concern
Looking at your description This could be fissure or fistula in anus, gluteal abscess or pilonidal sinus.
Probability of tuberculosis can't be ruled out with your description only, it requires physical examination and a battery of tests like blood for CBC, ESR, Liver function test and coagulation profile.
In my opinion you should.consult a general surgeon or proctologist for confirmation.of diagnosis and further course of action
Meanwhile avoid constipation and straining at stool
You can apply anaesthetic ointment like lignocaine for pain relief.
you should take lots of dietary fibers, fresh fruits and raw vegetables to increase roughage.

Please feel free to ask follow up questions if anything is not clear.
Best wishes
