L4-L5 facet arthropathy, disc desiccation & broad-base disc herniation slightly eccentric to the left extending into the neural foramina, there are modic type 2 endplate changes at this level, there is a small central annular fissure identified causing impingement on the thecal sac, there is mild-to-moderate left neural foraminal & left recess stenosis with impingement on the exiting nerve root within the left recess.
