Loose Motions Alternating With Constipation,abdominal Pain And Bloating After Meals. Low Hb Levels. Treatment?

Hi..I have 18 years old boy in my ward..He has 2 months history of loose motions alternating with constipation , abdominal pain and bloating after meals and incomplete evacuation..hez fatigued all the time..meal intake is also too low..hez pallor and emaciated..his Hb is 5.6..what should be my approach regarding his treatment?

Hello,

* The best initial approach would be a correction of vital elements of the pulse, pallor and abdomen pain with properly packed cell volute transfusion, GI-specific antibiotics, IV fluids as per the need.
* With planning to evaluate the condition with lab tests, ultrasound of the abdomen & pelvis; the need of CT scan and colonoscopy depends upon the turn about of primary concern.
