If I were your treating doctor for this case of lump in the breast since two years with pain I would come up with these possibilities , these includes.

Fibroadenosis may occur at any age after puberty . In this condition pain usually worse during
menstruation .
Diagnosis can be confirmed by
mammography and consult a surgeon and get his opinion.
2 Simple solitary mammary cyst , diagnosis can be confirmed by physical examination by a surgeon.
3 Benign cancer (
neoplasm ) of breast , however this is many type .
Here I would like to mention only
fibroadenoma either hard or soft ( as you are complaining that it is very hard ) . Diagnosis can be confirmed by mammography and physical examination by a surgeon.
4 Carcinoma breast ( least chance according to your history ) .
In my opinion consult a surgeon and get his opinion and physical examination will find out the exact nature of lump.
