All breast lumps need not be malignant (cancer). Usually breast cancer occurs after 40 years of age. Your friend is just 21 years of age and breast cancer at this age is very rare. And also breast cancer usually presents as single solid mass and also increases in size very raplidly. You have not mentioned since how long your friend is having these breast lumps. Breast lumps at the age of 21 and in both breasts are usually fibroadenomas. These are not malignant. If at all you want to confirm, you can get a mammogram of both the breasts so that it will tell us if there are any suspicious cancer lumps in her breasts and if there are any you can get a FNAC from those lumps to confirm.
Your friend can consult a surgeon and get the workup done for her breast lumps.
Good luck
