My Dad Has Been Hospitalized For4+ Weeks. 79 Yo Caucasion

My dad has been hospitalized for4+ weeks. 79 yo caucasion male with h/oCAD s/p triple bypass 25 yrs ago, a-fib controlled 3 stents, HTN on meds high cholesterol on lipitor, type 2 diabetes on oral meds. Started with URI sx that lasted 3 weeks on levaquin x3 days fever and SOB seen by pulmonary (no history of lung problems, except one previous episode of bronchitis a year prior. Pt was a 1/2 PPD smoker for 20 years but stopped 40 years ago. was admitted with dx of pneumonia treated with Rocephin and zithromax with worsening PO2. at pulmonologist CT scan showed effusion p02 on room air er 81%. progressive lung deterioration treated with high dose steroids improved enough after 3 weeks to be d/c'd to long term acute care on 7 L o2 to be weaned, at this facility oxygen was weaned to 1 l at rest 6 l with exertion, platelests dropped , required foley for urinary retention. clot in left leg no pain just warm, on xaralto. recommendations?
