My Ears Feel Like I'm Under Water.... Had It Checked

My ears feel like i'm under water.... Had it checked a year ago by family doc and ENT... Went to chiropractor for awhile and seemed to clear up. Now it's back... Also a couple years of trouble swallowing things like bread. It gets stuck going down. compensated by chewing a lot... some pain in the very middle of my stomach and inch above the belly button and the very bottom of the rib cage... Some on and off shooting pain at the top of my chest outward toward my arms. Some numbness and tingling in both arms sometimes... Today I had some muscle spasm in my face on the right side jaw. now that side feels slightly numb on my face and throat... When do I go to a doctor?
