My Husband Had A Stroke At Home, Fell And Hit

My husband had a stroke at home, fell and hit his head. he was taking Keppra 750mg BID. He was also on xarelto for afib, he had a pacemaker. He had ran out a week prior and was taking baby ASA 81mg.
He was given TPA at the hospital. CT showed after theey gav it to him he had another stroke and a heart attack. As a nurse of 26 years my instincts say they should not have given the medication, and as his spouse and caregiver, I am just trying to make sure Im not looking for someone to blame in his death. No autopsy was done.
