My Question Is A Lengthy One. My Problem Started Around

My question is a lengthy one. My problem started around new years 2015. I had an ear infection and it took me 3 weeks to get in to see my family physician. He prescribed me Sofracort twice daily for 4 days, which didn't work. It just made my ear burn and hurt worse. Within a week my right tonsil was itchy and bothering me so I went back. My physician said I had a peritonsillar abscess and prescribed me clindamycin. I finished the clydamycin. I felt better for about a week when the pain in my ear and tonsil came back. I went back again and was told there's no signs of infection but he gave me more Sofracort in case it was inside my ear. Again it had no affect. So I went to the ER and was rudely told there was nothing wrong but I started crying so they referred me to an ENT specialist. I saw him and he said I have no wax in my right ear and to stop touching it and no more medications. I insisted that something was wrong so he looked again and said my teeth are bad and my jaw clicks and pops which suggests TMJ dysfunction. I did what he said and I left my ear alone and focused on my teeth. The entire time I was still having ear pain and my ear was extremely itchy. Especially in the morning. Along with clear fluid and sometimes it got crusty. The pain was around a 4 out of 10 most if the time but would jump to 8 or 9 randomly. I woke up one morning a couple month ago with a lot of pressure in my ear and it felt like something was in it. Being half asleep I stuck my finger in there and scratched at it. And noticed blood. Enough that it was running out of my ear. So I went to ER again. I was told that I had a middle ear infection that perforated my ear drum so it was bleeding. I told them my situation and they said my ENT was wrong for telling me to leave it alone. And prescribed me ciprodex and amoxicillin. I took and finished them. Again within a week or 2 my symptoms came back. Recently my ear has become worse, my tonsil gets pain like someone is jabbing it with a needle. My throat is sore again, my lungs ache( I am a smoker though), I have sinus pain around the bridge of my nose, around my right eye, right cheek bone, around my right ear and down my jaw bone on the right side. I can't figure out if the ENT is right and it's my teeth. My teeth are rather bad on the right side and front. I have multiple fillings cracked and falling out and a few new cavities. But my teeth never hurt the whole time until recently though. My question is, is it possible that my ear infection spread and has moved down to my teeth? Or is it more likely to be the other at around? Ear problems run in my paternal side. 4 cousins have had tubes put in and 1 uncle is legally deaf now. I just don't feel like this is all because of my teeth. And I really can't afford or have the time to see a dentist.
