My Son Is 20 Months Old. He Was Given Rocephin

My son is 20 months old. He was given rocephin shots 3 days in a row for reoccurring double ear infections. The second day he was given the shot. The nurse did not hold him down well and he moved. The injection site had a huge bubble underneath his skin and his leg was bleeding, his skin was torn, she scratched him and cut his skin when she pulled the needle out. It was almost as if she ripped his muscle with the needle. It's now almost 4 weeks later and my son is complaining of pain in his leg and he had a knot in his thigh. Is this normal with this type of injection? He is sick again and running fever so it's hard to tell if it's from whatever illness/cold/virus he has or if it's from the injection site.
