My son was a patient of a Dr. Horowist at a psychiatric facility in San Antonio, Texas in October 2014. I am trying to find the name of the hospital he stayed at. My son is a 4th year medical student and needed to take a medical leave. To get reinbursed for part of the tuition I need documentation to validate his medical leave. I need to get a form for release of information from the hospital. The hospital was near a Holiday Inn. Do you have any way of helping me get the hospital name and address. Genevieve Housen
