My Wife Has An Enlarged Left Ventricle, EF Of 20%,

My wife has an enlarged left ventricle, EF of 20%, and now they are telling us that she has PVCs, Bigemini.
We thought that she was going to get an implanted defibilator, but now they are saying they need to address the PVCs first. We are confused. A lot of what I am reading on line sounds like the PVCs are not that serious of a problem. What should I be concerned with most? The PVCs, or the low EF?
