Need Medication For Diabetes Mellitus

My father is 86 years old and was treated for HT with Diabettes mellitus. He has also got slip disc and used to wear belt as support for walking. Slip disc was detected more than 15 years back and Neuro Surgeon in Mumbai decided not for surgery. In Nov-13 he fell down from chair while writing. He was unconscious for 1-2 minutes coupled with passage of stool. On undergoing MRI the Impression revealed Age related Atrophy with old Ischemic changes. Study showed CSF and VR spaces prominent. Multiple T2 and flair hyperintensities are seen in bilaterial periventrcular white matter pons and left thalami S/O old ischeamic changes. Venticular system is prominent with perventicular hyperintensities. Ni midline shift seen. Cerebellum appears normal in signal intensity. Fourth ventricle is normal. The effect of the fall was that he stopped walking and there was loss of strength in the legs due to which he was not able to stand up on his own. Now (in April-14) after 3 weeks of physiothereaphy and medicines that include Clacix, Tab Attorva (10) & Tab Strocit 500 - 1 BID coupled with Injection Vitrofol - he has been able to move on the three leg support system. Kindly advise any other treatment for him to improve this situation as he quite often talks all irrelevant things and also gets uncontrolled angry time and again. He is also adminstered Tab Olanzapine 5 mg daily.

Diabetologist 's  Response
Hi, welcome to our site. I am Dr Saumya Mittal, MD.

Read your query. That is a very significant question and i appreciate your problem. I will try my best to answer your query

To administer and suggest the medicine for diabetes, I need to know the sugars that he is having. Now your query gives a great deal of details about everything that has happened, and the medicines he is on, but remains silent on the sugars he has and the medicines for diabetes he is on, if any.

I would suggest that you let us know the details so we can help you further.

And watch the olanzipine. It is known in some cases to cause an increase in the sugars. Also requesting a kidney function test, to assess if the need for insulin may be a better option

I hope this helps you. Inform the reports mentioned above/if any other so I can be of help further.

I have given you the answer to the maximum considering the information provided. The results of the tests could further enhance my answer to you.

Please do understand that some details could be extracted from a detailed history and examination.

Please feel free to ask another query. I would be glad to help you. Looking forward to your return query with the details asked so that I can help you further.
(If the answer has helped you, please indicate this)
