Ok I'm Currently New To ICU (9 Months) I Was

Ok I'm currently new to ICU (9 months) I was pulled from my normal ICU to another ICU floor; I had 2 patients a post TPA stoke patient & new GI BLEED Admit.

Well the stroke patient was neuro check 1hr & manual BP until 10pm; at 10pm I had to take the patient down to CT because he was it was within 24hr of TPA; the CT reveal a midline shift from left to right; 9mm so I notified NEURO n she told me to call the attending & see if there's anything they want to do. Well the resident order 3% sodium chloride.

I noticed a change in the pt neuro status & I alert my charge nurse we assessed him & I called the MD & they order another CT; which reveal 14mm herniated bleed collapse ventricle; notified LSU as well as radiology. From there nothing was done besides intubation.

Well my supervisor said that my charting was off & that it look like I took 2 hrs to call the MD about the neuro change. (All this happened on Tuesday night)
Well the pt family made him a DNR n he passed because they said he was brain dead. But the neuro MD said they think the could of save him if they would have did a procedure. But no one made that decision after the second CT was done.

So now my supervisor is saying that it may go to court n I'm get charged with negligence. Is this possible? Can I be charge? I couldn't make the MD decision I informed them of the changes even tho my documentation was off by the time. After the second CT, could the MD do a procedure to decrease the pressure after the second CT?

Pt 60yr old male with 2 strokes in the past. Hx: HTN, DM
