On Ventilator After Suffering From Heart Attack And Lungs Filled With Fluid. Should Allow Tracheotomy Or Remove Ventilator?

Two weeks ago my wife had. Heart attack and echo showed function at 20%. Catheterization scheduled but before doing lungs filled with fluid in 20 minute span. After cath heart function down to 15 and on ventilator and balloon pump. Heart pump taken off. Three days later but cannot remove ventilator. When turned off breathes jump to 45-60, volume drops to200-250. Now they are asking me to decide whether a tracheostomy should be done. All arteries to heart are clogged, 2 100%, others 90% or more. Should I allow trach or remove ventilator and see if she will stabilize? Does not appear she is candidate for surgery due to extent of damage.

Very Sorry for you are facing such a difficult situation.
Her present condition is very critical.
1. her heart function is 20%.
2. She has TVD with 90-100% stenosis ,on ventilator for 2 weeks.
probably major part of myocardium is damaged and is not going to get benefited with surgery, also surgery is very high risk.
If you will remove ventilator, she will not survive for long.For prolonged ventilation, tracheostomy is recommended.
So, in present scenario please give he consent for tracheostomy and pray for improvement.
