PPA, Irregular Heartbeat, Little Rip In Heart, Sweating, Tiredness. What Could Be This?

Hello, my name is Sheila. I have PPA, irregular heartbeat, and a little rip in my heart. Lately, my spouse and I are walking our dog when we do it seems it don't take long for me start sweating in my back than my head plus get tired. So I sit for a bit to renew my strength and test my sugar which is 96 tonight but don't what is happening:/

It might be very necessary to exclude a possible serious heart condition. Consulting a cardiologist for a complete clinical review is important.
