It seems that you are having fungal infection on the parts.
Go for anti fungal cream local application and anti fungal tablet orally,one tablet weekly for one month.
Another possibility is that due to not having proper local hygiene there is collection of smegma on the glans causing itching and soreness.
Clean glans averting the fore skin daily while taking bath.
