Thank you for your question. Vaginal
hysterectomy can be confusing for some patients during recovery because no obvious
incisions are seen. However, there is an incision at the top of the vagina where the
cervix previously resided. Most surgeons attach the ligaments at the corner of the vagina after removing the uterus and cervix, while closing the vaginal cuff. The ligaments involved are the
uterosacral ligament which attach from the sacrum (lower back) to the uterus, the round ligaments that run into the
inguinal canal and the utero-ovarian ligaments.
While recovery from vaginal hysterectomy can seems less intense, it is still a major operation from which it takes about six weeks for full recovery. Significant activity may place tension on these ligaments, which would feel like a stretching/burning sensation in the lower pelvis.
You should follow up with your operating surgeon and describe your symptoms. Decreased physical activity is still in order with a major operation, even though incisions for vaginal hysterectomy are not visible.
