Pressure In Perineum Area With Intense Anal Pain, Constipated. History Of Prostate Infections. What To Do ?

I'm 43 years old. I lift weights regularly and I have history of prostate infections atleast twice in my adult life. Currently, I have pressure in my perineum area that I can't distinguish from hemroids or prostate issues. The anal pain gets most intense when I lay down at night but I also seem to have muscle contractions in the perineum. The aching can reach as far as th base of the scrotum up under the testicles. This began about a month ago then two weeks ago I passed a kidney stone. I've had hemroids before but there is no blood to be found recently. When I passed the stone two weeks ago the doctor put me on a 1 month dose of Cipro for a prostate infection (No tests run) that I'm two weeks into now. I'm experiencing slight constipation which I'm sure is related to the Cipro and that's not helping things. I'm having a urinary urgency but I've been drinking more water than normal to stay hydrated for the Cipro.
I'm very concerned and I'm really not sure whether I should just let the Cipro play out or if I need to see a specialist. If so do I see a Urologist or a Gastrointestinal physician first?

Hi there,

Thanks for writing in.

It is a good thing that you are drinking adequate water to maintain your hydration.

In addition I would also suggest that you take fresh fruits and vegetables, whole grain cereals and fibre in your diet. Exercise 30 minutes a day 5 days a week.

Both exercise, good diet and water should take care of the constipation.

The anal pain or pressure in the perineum might be due to proctalgia fugax.
Other possibilities I can think of are thrombosed hemorrhoids, anal fissure, prostatitis etc.

In spite of following dietary measures and exercise, if there is no improvement, I would advise you to visit a gastroenterologist.
