Hello and welcome to HCM, The symptoms of spontaneous leakage of semen, lower back ache and pain while passing urine are suggestive of infection. The symptoms indicate that you may be suffering from prostatitis. Prostatitis is inflammation of the prostate gland. A urine examination -routine and microscopy is required to look for evidence of infection like pus cells and positive esterase test. If there is evidence of infection, culture and drug sensitivity need to be done to select appropriate antibiotic for the infection.
Thanks and take care Dr Shailja P Wahal
