Abdominal condition of yours is due to Inflammatory Bowel Disease (IBD). Colonoscopy 3 years back have confirmed it with colitis. You should go for a review now to see if complications have arose. Further treatment will be based on complications, if any. Sometimes liver problems also lead to irregular bowel habits, so it can't be ignored.
