Right Flank Pain, Lower Abdominal Pain, Painful Urination, Fatigued Sore Muscles And Joints, Metallic Taste In Mouth. Thoughts ?

11 yr old girl presents with rt flank pain, lower abdominal pain, painful urination, fatigued,sore muscles & joints. Itchy mouth, metallic taste in mouth, headaches, sore throat (no strep) and lower jaw chatters sometimes (not cold). Sono of pelvis & kidney normal. Ct scan of abdomin showed normal. No bacteria, urine microscopic performed ("), alkaline phosphatase 290 (more than double the high range), chloride slightly elevated, urine mucous occ("), MCH & MCHC slightly elevated, WBC 4.30 slightly low.

Hi ! Your symptoms are pointing towards a urinary tract infection, though it has not shown itself in the urine test. You are suggested to go for a urine culture sensitivity test before going for any kind of antibiotic. Chronic urinary tract infection may give rise to fatigue, sore muscles, headaches due to associated fever. The infection might be hematogenous from the sore throat. If she has taken any antibiotic for the sore throat, then no organism might be seen from the throat swab. Also, let her take a lot of liquids. If the symptoms persist or increase, then you should get your patient reviewed by a paediatrician, or a urologist.
