Hello ma am. I am sorry to inform you that there are multiple abnormalities detected in you scan, let me start from the basic. You uterus was found to be bigger in size than a normal uterus, usually seen in pregnancy or various conditions (yours is due to various conditions). There were 3 fibroids found kn your uterus of differing size and location which will have to be either removed through procedures like dilatation and currettage, electrocauterization, etc; or if you have reached an age close to menopause or if you have completed your family, your doctor would suggest the most effective way of treating fibroids, which is with the removal of the uterus (hysterectomy). Both your ovaries were also found to be enlarged, cause to be further investigated. There was free fluid found in the pouch of douglas which is the site where fluid or blood can accumulate in conditions like a rupture of the ovaries, fallopian tubes, ectopic pregnancy, etc. All these findings suggest the best treatment to be a hysterecromy.
