Severe Chronic Shoulder Pain. Anti-inflammatory Drugs And Exercise Offering No Relief. What Could Be The Underlying Cause?

Hi Doctor,

I am writing this email in hope for a help. My wife is suffering a lot with severe shoulder pain since more than an year. We have been to multiple doctors in United States but there is no help, All doctors are saying that she is absolutely fine and the pain will go away in some time after regular exercises.

She is regularly doing all the shoulder exercises but there is no improvement in her pain relief. She is just 28 Years.

She played tennis last year for only once and since then this is the case. Doctors initially said its a muscle pull/tear and inflammation. They suggested anti inflammatory drugs, she was fine when she takes that but pain was coming back if she stops. She was also recommended for physical therapy, we have been to therapy sessions for 6 months but when we give a gap in therapy pain is coming back again.

We stopped visiting Physical therapy as its not giving permanent solution , she then started doing regular shoulder exercises at home and hoping for a relief. But not much help still.

We both don't know the root cause of the pain , and its a really a concerning part that the pain is still there even after an year. We are skeptical about something big going on in her body.

Could you please take a minute or two to understand my wife's situation and suggest the treatment.

Thank you for posting your question. I will do my best to help you

I have tried to understand your query to my best.

I have also understood that you are well aware of the illness (Chronic shoulder pain) in details as per the information provided .

Playing tennis has been mentioned .Sprain are common & can occur even if played once or during any activity accidentally .

It could be Frozen shoulder or Rotator cuff injury or sprain as it is chronic in nature .

If symptoms persists for more than 12 to 18 months ( Despite supportive treatment -physiotherapy,painkillers etc) ,Then may need Steroid injection into the joint capsule may be required for improvement.
In a small percentage of cases, surgery may be needed to loosen the joint capsule so that it can move more freely.

Imaging Tests -X ray and MRI Test are usual done to rule out other structural problems if suspecting.

However,Since the duration is more than a year now, Advise to consult Orthopedics again to evaluate further.

Also continue the pain reliving medication till then along with excerices.
