In terms of chest discomfort or pain, the more cardiovascular risk factors you have: advanced age, first degree family members with early heart attacks or stroke, hypertension, diabetes, smoking, high cholesterol, obesity, sedentarism, stress, etc, the more concerned we are as physicians. Chest pain arising from the heart could happen both with exertion or at rest. Itching would be a highly unusual manifestation of heart disease. Nevertheless, if you have any of these risk factors, then it might be reasonable to obtain a stress test to clarify.
