Should I Be Concerned With The Infant Losing Weight And Having Bowel Movements Yellowish Liquid And Is Breast Fed And Hardly Eats?

I care for a 14.5 month old boy, and have concerns about his bowel movements. He has allergies to dairy and nuts, and drinks soy milk and is still breastfed regularly. When he is at my home, he drinks next to no soy milk, and typically eats only fruit. At home, it sounds like he hardly eats, and his mother breastfeeds him typically all evening/night/morning before he comes back to our home. He has lost some weight, it appears, and his bowel movements are consistently yellow and pure liquid. This has been going on for over a month. Is this cause for concern?
