Should Dosage Of Metroprolol Be Reduced For Severe Fatigue And Dropped BP?
I am 75 yrs old.I am presently taking 95mg Metroprolol CR at night & 2.5mg Plendil ER in the morning for B P. If I work outdoors in the heat of afternoon , 24C, I feel very tired after about an hour and when I check my BP it always has dropped about 15%. Will dropping the Metroprolol to 47.5 mg help?
The need to adjust the medication would depend to the level to which the blood pressure drops and any symptoms that occur with the reduced reading. The frequency that this occurs is also important, if it happens only rarely then the medication does not need altering.
It would be best to speak to your doctor before adjusting any of your medication. Decreasing the metoprolol dose by half may result in a sharp increase in your blood pressure readings.
Metoprolol is the preferred drug for treatment of hypertension when it is associated with coronary heart disease and congestive heart failure. For treatment of hypertension alone, it is not first line of drug . Considering your age ,I feel that metoprolol may not be ideal drug for you provided your are not suffering from the above mentioned associated illnesses . Drugs such as amalodipene may be more suitable to you .However if there is compelling indication for use of metoprolol I suggest that you can reduce the dose of metoprolol to75 mg. You can not break the tablet metoprolol CR into two halves and take ; it will produce erratic level of the drug in the blood as it is a controlled release tablet. Further, suddenly decreasing the dose of metoprolol may lead to irregular heart beats and rarely death may occur. Hence metoprolol may be used in a reduce dose as 75 mg; tablets containing 25 mg can be taken three times daily.
