Hi Mary Jane,
thanks for asking in Healthcare Magic.
Q tip has caused laceration in the external auditory canal (outer ear canal). Since you don't have loss of hearing, it is highly improbable for you to have a ear drum perforation. However, there may be an injury to ear drum without perforation or injury to the outer ear canal. Hence you are having blood and serous discharge.
If it is an injury to the ear drum, then yes, it is serious. Because, if it is left unattended or if there is further trauma, it can result in ear drum perforation leading to impaired hearing, pus discharge associated with pain (Acute Suppurative Otitis Media or ASOM).
Antibiotic ear drops like Tobramycin, anti inflammatory drug like Ibuprofen and oral antibiotics like Clarithromycin for a period of five days will help in the healing of wound.
I hope this is useful to you.
