Stomach Ache, Appetite Loss, Anxiety, Stomach Heaviness, Constipation. What Is The Best Treatment?

hello,patient 76 yrs male,after by pass surgery 10 yrs ago,he cant walk properly bcoz of acetabular fracture from last few months,now he has stomachache,heaviness on the stomach,loss of appetite,waekness, anxiety & constipation. having symptoms for 10-15days.H.Pylori- 7.26 arbU/ml. He is getting treatment- amoxycillin , Isameprazole, clarithromycin kit, digestive enzymes(pepfiz).cyproheptadine & antacid . taking this treatment for 3-4 days but still no difference.he doesnt WANT TO GO THROUGH ENDOSCOPY. Plz suggest what is best treatment in this case for the loss of appetite & weakness as he doesnt feel eating,now he got severe weakness as well.Patient is from India,im his son an orthopaedic surgeon ,staying abroad so plz suggest what is best.thanks

Hi.

Noted that you are an Orthopedic Surgeon and history is about your 76 year old Father.
As you have mentioned he has developed anxiety which can be the main reason for IBS sort of symptoms.
But it is necessary to rule out any pathological cause for the loss of appetite, pain in abdomen and heaviness. This itself can cause constipation. This along with fracture of acetabulum needs a proper stool softener and laxative to tackle constipation to start with.
I would advise him the following:
Consult a Gastroenterologist for clinical evaluation, examination and go for all relevant investigations:
Upper GI Endoscopy.
CT can of abdomen.
Tests of stool, blood and urine.
Any other tests as needed.
All these things will lead to a proper diagnosis allowing a specific treatment.
If all the tests are within normal limits then this may be treated as IBS.
As you know proper management of ecetabular fracture is a must considering his age.
