Suffer Hirsutism, Overweight, Increased TSH, Irregular Periods, Really Hairy. What Is The Reason ?

Hi. I'm a 17 y/o girl and for years now I have been struggling with hirsutism. I am overweight (I have 98kg/ 175 cm – 215,6 lbs/5’9’’). At first, I thought my hormones weren't okay, so I checked my thyroid gland hormones (TSH : 1,8 mlU/L ; FT3: 4,4 pmol/L; FT4: 15,3 pmol/L), testosterone (2,4; 2,88; 3,08; nmol/L - I did it three times, and normal level, according to this test is 0,3 – 3,4 for women and 9,4 – 33,5 for men), cortizol (410 nmol/L), ACTH (47,3 pg/mL, normal lvl: 50 pg/mL) FSH: (8,28; 8,65; 3,64), LH (3,62; 4,49; 10,70), progest. (1,61; 0,82; 0,71), estradiol (0,171; 0,114; 0,097), prolactine (285; 290; 210My periods used to be pretty regular (every 4 to 6 weeks), until this year. I had my period normally in January, but after that, I had it again in April, after taking dydrogesterone, and again in July, after taking the same pills. Now, my endocrinologist said I have PCOS, but my gyno says that I have no “proof”, because my hormones are okay, so I really don’t know what to do. My biggest concern is that I’m REALLY hairy, and I’m not exaggerating. I have hair on my arms, around my nipples and chest, below my belly button . I also have really hairy thighs, fingers and toes, not to mention my moustache, beard and side burns. I am really ashamed to wear a ponytail or go to the beach in a bikini (not because I’m fat). I have visited a gynecologist and endocrinologist and both of them said that I need to lose weight, and I am working on it (I used to weigh 103 kg). They say when I lose weight, my hair isn’t going to be visible or anything, but I doubt it, because my family is pretty hairy – we’re Mediterranean, I guess that’s the reason . I wanted to take birth control pills, because I heard they help with excess hair, but my gyno doesn’t want to prescribe them, until I lose weight. I have a class trip in about two months, and I was wondering could I epilate or wax this hair, and will it cause my hormones to go crazy (crazier, that is). Also, do you think I have PCOS? I know this is really long, but I hope you will find some time to read it and answer my questions! Thank you so much! yyyyy
PS I forgot to mention I’m taking metformin 850 mg, 2x/day, even though my blood sugar lvl is okay – 5,0, normal lvl: 4,1 – 5,9. Bye!

General & Family Physician 's  Response
Hello,
Thanks for the query to H.C.M. Forum.
Let me explain you few symptoms of P C O S ( also known as PCOD i.e. Poly Cystic Ovarian Disorder or syndrome).
1 Androgen excess manifesting as hirsutism and acne.
2 menstrual irregularities , dysfunctional uterine bleeding or amenorrhea ( no bleeding )
3 obesity with glucose intolerance.
4 alteration in L H/ F S H ratio.
5 Elevated serum testosterone level.
6 Ultrasound findings , of atretric follicles with increased ovarian stroma.
In my opinion your testosterone level is high according to Harrison test book report.
Normal level 0.21-2.98 n mol/L.
L H / F S H ratio is also in alteration .
Esradiol hormone is low according to Harrison .
Normal follicular phase 184-532 pmol/L
MID CYCLE PHASE 411-1626 p mol/L
luteal phase 184-885 p mol/L.
Estradiol reduced level, changed ratio of L H / F SH , testosterone elevated , hirsutism acne all are suggestive of P C O S.
So please consult a gynaecologist and get one more opinion ans treatment.
In my opinion this is not complete treatment , as mentioned by you in query.
Good luck.
Dr. HET
