Suffering From Acne, Appearing Weeks Before Periods. Taking Isotretoin, Condition Improved, Worsened. Suggest Ointment?

Hello doctor,
i am having acne again of large nature.I had oral isotretoin medication six months back and things were under control. but this sudden onset is depressing with scars that remain dark even after 4 weeks of onset. my periods are regular and just weeks before menses I have acnes.

i would like you to advice which topical oitment i need to get rid of the scars.

hi,
since your acne got improved initially and worsened which is very common. once you concieve, these problems fade off or reduce automatically.

for now, you can take course of antibiotic along with this azithromycin 400mg once daily for alternate days for 4 weeks, in addition to your isotretoin. also, use acnil bar soap 3-4 times a day, to wash your face. you can use clindamycin gel also.

hi,
thanks for the query. the large acne that you are having are basically nodulocystic acne. usually such kinds heal with scars and hyperpigmentation. also i would like to highlight that it is normal to get premenstural flare ups.
first of all you need to do your hormonal profile and serum insulin levels, accordingly you can be given hormonal pills for the same. along with this you should start with oral isotretinoin since you showed good response last time. then add oral and topical antibiotics to your regimen.
however getting your hormonal profile should be your first step. as far as pigmentation and scars are concerned there are many treatment modalities including peels, lasers, rollers, fillers.
