Suffering From Acne, Given Demelan. Does It Cause Hyper Pigmentation ?

Sir, I am Dr.Deepthi, i have been suffering from acne for the past 6 months, i am a known case of PCOS, my dermatologist has put me on azifast 500mg for 8 weeks, Aldactone 150mg, T.Yasmin, T.Sotret 20mg, i had also got 4 salicylic acid peels done, though my acne has come under control and i am having only fewer breakouts, i hav PIH on my cheeks... i hav been given Demelan for it... but i hav noticed not too much difference with it ( been usin it for 2 months now) and also i was wondering if it would cause hyper pigmentation because i noticed a small patch on my face which has become a little dark( not from PIH), kindly address my queries...thank you.

Dermatologist 's  Response
Hi, Dr. Deepthi welcome to HCM.
Demelan has 10% glycolic acid, 5% arbutin and 2% kojic acid, all three of which are hypopigmenting agents. Glycolic acid accelerates hypopigmentation by exfoliating the skin as well. None of them are implicated in causing rebound hyperpigmentation.
Although irritation with glycolic and kojic acid has been reported, onset of it after two months is counter intuitive.
So your hyperpigmentation doesnt seem to be triggered off due to demelan to me. But you could try tapering it and observe the patch for a while.

As for the slow response of PIH, if you are unsatisfied with it, you can go see your dermatologist and get a prescription stronger depigmenting agent like hydroquinone, or glycolic peels.

Hope it helped. In case of any doubts follow up questions are most welcome.
