I would come up with these possibilities , these includes.

Appendicitis chronic sometimes did,t visualize in ultrasound , diagnosis can be confirmed by physical as well clinical examination by a physician.Get in blood examination for leucocytosis, e s r.
2 Next reason is renal involvement as there is
hydronephrosis ( uretero ) present , this is evident by SCAN. This is the cause of pain as in this condition fluid accumulates in kidney and ureter causing the pain on right side.
3 If you are a FEMALE patient may be due to
ovarian cyst , diagnosis can be confirmed by
ultra sound as sometimes a very small size cyst don't visible .
In my opinion first of all consult an
urologist and get his opinion ( 2nd).
