Suggest OTC Medication For Rashes Under The Breasts And Folds Of Skin

what treatment over the counter can I use to treat rash under my breast and in the folds of my skin. I am a large person that has to take prednisone because of Bechet s disorder and otherhealth problems. I USE A POWDER WITH zinc,and all the prescription creams don t work. I have tried a cream for diaper rash and it works great,but messy. doyou know of anything else that would help. The itching ,burning, redness, sweating and smell is annoying,i cannot wear a bra.

Thank you for your query.

You seem to have fungal infection (candidiasis) associated with a dermatitis. Zinc oxide/diaper creams helps by reducing inflammation and soothing irritated skin.

I would first confirm the diagnosis by examining you. If is is candidiasis, I would give you am antifungal ointment - Clotrimazole with betamethasone
Clotrimazole is an antifungal and betamethasone is a steroid that helps with the itching and redness associated with candidiasis.
As you are already on prednisone, I would advice you to use the combination therapy only for the first few days and then switch over to Clotrimazole only preparations.

How to apply?
- first clean and dry affected area. Once dry, apply the ointment over the affected areas enough to form a thin(should be able to see affected skin underneath) white film over the affected area and about an inch outside it.

- apply twice a day (one of the times before bed).

- do this for 2 weeks or till skin lesions disappear, whichever is later. Once the itching has reduced to about 60%, switch from combination therapy to plain Clotrimazole.

-reduce the frequency to once at night for a week and then to once every alternate day for a week and then stop it. This slow tapering is important to treat the slow growing and equally slow regressing fungus.

Tips:
- when at home try to air out the affected area.
- wear breathable material clothes and change as soon as it is soiled.
-use of Clotrimazole powder about 2 hours after you apply ointment in the morning will help absorb any sweat.
- use small thin cotton cloth to keep under the breasts or folded area. Keep changing these every two hours. This way there is minimal moisture and the cloth feels fresh on the skin and prevents irritation.
- bathe in cool water to relieve itch, inflammation. Ice packs enveloped in a thin cloth, applied for 10-15 minutes also helps.
