Suggest An Alternative Treatment For Psychosis For My Grandma

My grandma has acute onset psychosis. We stopped giving her her steady dose of budesonide (steroid) a month ago. Overnight, she became paranoid, aggressive, basically delirious. She was in the hospital for a week following stroke-like symptoms (no evidence of stroke, all labs clean). She was prescribed Haldol in the form of drops, but the pharmacy said it does not exist as drops. She is currently actively paranoid and refuses to take the pill, without which her behavior is unbearable. What do we do?

Hi,

It can be acute psychosis though other causes like CNS infection and sepsis should be ruled out. Inflammatory markers, cultures and lumbar puncture or MRI may be considered. Steroids induced psychosis may be still most likely but it's a diagnosis of exclusion in such case. During spells of extreme irritability haloperidol can be given in form of intramuscular injection if she is resisting oral dosing.
