General & Family Physician 's  Response
Hi,

Based on the history you have shared, if OTC analgesics like acetaminophen or Ibuprofen are not effective alone these can be used in combination alongwith more potent analgesic like tramadol.

Nerve pain suppressants like gabapentin or duloxetine can also be used after a neurosurgeon review.

I suggest taking fresh MRI and discuss for surgical options is local remedies or blocks had failed to yield results.

Hope I have answered your query. Let me know if I can assist you further.

Take care
