I have a 65 y/o female patient w PMH recurrent proteus mirabilis UTI, DM II controlled, Right residual hemiplegia, HTN controlled, obesity, homebound patient. She was treated w Rocephin IM first followed by Gentamycin IM. Urine Cx still positive w more than 100,000 col. Could you please let me know the antibiotic that you selected for her?
HI Well come to HCM I really appreciate your concern, Urinary tract infection in female patient is challenging, if the case is associated with other medical condition as it is being stated here then it would be more challenging. Ideally giving antibiotic in Urinary tract infection without culture sensitivity test is not advisable, the test rule out the possibility of infection due to organism because it may not be the organism that could cause the symptoms, second administered antibiotic without the test could cause the resistance, in geriatric patient great care need to be taken for urinary symptom particularly in female patient it may not be the infection but the whole urinary system need to be taken under clinical consideration, the most important is source of infection it is to be found out. hope this information helps.
