Suggest Appropriate Therapy For Stress And Anxiety

I have had a problem with stress and anxiety for many years now, more than 10 I would say. I am afraid of everything, especially when it comes to my relationship with my partner. I feel great on one moment and the next I am worrying about what will happen? should we be together? will he hurt me? Am I making the right choice? Should we move in together? Etc... I have anxiety about a lot of other things to, not just my relationship. I wonder if I am taking the right path, like I cannot trust myself to make the right choice. Any type of advice would help.
