Hello dear, I understand your concern.Though your head injury did not make you lose your consciousness and so it does not mandates you to go for computerised tomographic scan (CT scan) of head. However, you are feeling dizzy, having black outs and severe pain and you apparently think that your injury was not mild.Taking into consideration the above facts,I advise you to visit a neurologist to get yourself thoroughly examined and get a CT scan of head to rule out any intracranial injury(if your neurologist agrees with it rationally).I hope it helps you.
