Suggest Dosage For Cilacar

Dear Dr. Matthew Mintz, I was diagnosed with ESRF due to Hypertension, and had a Renal failure in 1990 and had a Transplant done in 1991 March 5th. I was taking Hypertension medication for several years and continue to do so, I was on a regiment of 6am Aten 25mg, 9am Telma 80mg, 2pm Aten 25 mg, 5pm Amologuard 2.5mg, 10pm Telma 80mg and SOS Amologuard 2.5mg. I visited another Nephrologist recently and was advised to change to: Stop Aten 25 mg completely and take Telma 80mg, 1-0-1 take Amologuard 2.5mg 0-0-1, and in case the pulse is above 80 and BP above 140/90 then take a MET XL 25 1-0-0. As I was having mild chest discomfort I visited a Cardiologists and he has told me taht 80mg of Telma twice a day was not advisable and changed to the following: CILACAR T 10 + 40 1-0-1 (this has a combination of Clinidipine & Telmisartan) 1-0-1 Nondon 2.5mg at 5pm (Nevibolol) q.d. 5pm But since my BP dropped to 110-75 I stopped Nondon 2.5mg and continued with Cilacar T b.i.d however my BP keeps dropping to 110-75 and makes we tired and lethargic and afraid that I could get dizzy. I would like you suggest if I should take a lesser dosage of Cilacar T as this contains 10mg of Clinidipine whereas just 2.5mg Amolguard was all I was taking before. Now that I have stopped Aten 25 mg and stopped Telma (Telmasartan) kindly let me know if I can take Cilacar T (5 Clinidipine and 40mg) q.d. at 6am and then 10pm Nondon 25 mg. Awaiting your reply. Hello,

If your BP remains around 110/75 mmHg, then it is actually good level. The normal BP range is 100 to 140 / 60 to 90 mm Hg. Considering that 115/75 mmHg is absolutely fine. That level does not lead to any symptoms which you have mentioned. There could be other reasons for feeling lethargic and tired. You may have to look at your hemoglobin level too.

Or any other related issue that might be contributing for such symptoms. In fact compared to Amlodepine Clinidipine is a better molecule as it does not cause pedal edema(swelling of lower limbs). So the present drugs given for your hypertension control are really good as apart from reducing the BP they also help reduce load on heart and help kidney functions. Thus, please continue the same.
