Suggest Effective Alternative Medication To Hydromorphone

Hi. I live in chronic pain. Dishes tommy, 10% of disc left btwn L3/l4, etc. I have been taking 3 X 80 + 3X 20 mg oxy neo with 20 mg oxy Ir for breakthrough. My cost of oxy neo is 1500+ for 270 pills. I need to change. Dr is thinking hydromorphone conversion. What would the equivalents be in hydromorphone family of drugs?
