It seems that he might be having yeast infection on the part giving this problem.
Go for one course of oral anti-fungal medicine like Flucanozole 150mg once in a week for 4 weeks.
Give him Cetrizine daily one for itching.
apply anti-fungal cream locally.
Keep local hygiene clean, dry and airy.
Ok and take care.
