Hi,
The patient might need surgery for any fistula as you suggested, and if in lungs it can be any abnormal connection between lung and pleura, but since bed ridden patients are prone to repeated chest infections or urinary tract infections specially with catheterizations so that might be the case and your patient may need antibiotics like ciprofloxacin or dependant on urine cultures, which should be done before starting antibiotics. The catheter needs to be replaced as well.

Hope I have answered your query. Let me know if I can assist you further. Wishing you a speedy recovery.

Regards,
