Suggest Remedies For Persistent Pain In The Knee

I have had pain in my right knee for 3 years. The pain requires me to use a knee wrap at times. The pain is below my knee cap. When I stand up it sounds like ground up glass or sand in my knee cap. At times the pain is moderate & then flares up to severe. What is going on?
