Suggest Remedies For Recurring Painful, Swollen Veins In The Forearms

I have recurring painful, swollen veins in my forearms. I have had thrombophlebitis in the past, but the urgent care doc told me it was just a bruise. I have been on Coumadin for over 20 years. I know these painful veins are not bruises (because I bruise a lot and these are not the same), but if it is not thrombophlebitis, what could it be?
