A 56 year old woman presents to the emergency department complaining of severe pain in her lower jaw and neck that has developed over the past hour. The pain is not sharp, and it is not relieved by rest or by changes in position. She took Ibuprofen when the pain started, but the medication provided no relief. She also complains of nausea that began shortly before the onset of jaw and neck pain. On further questioning, she admits to a “heavy feeling” in her chest, which she describes as squeezing or crushing sensation. She is profusely diaphoretic.
First the tone of it is not as if it is a personal question and therefore this is highly inappropriate. Second, we do not diagnose, treat nor prescribe to patients we don't see so this is an informational service only. Third, you are going out of your way to make it a classic heart disease presentation in a woman but there are some other possibilities including indigestion (which is probably statistically more common than heart disease even when the presentation is "perfect" for the heart). drug side effect especially if she uses NSAIDs on a regular basis would cause indigestion--possibly even fatally so if there is bleeding, panic attack if there is any vertigo, that opens a lot of other possibilities for brain or ear conditions. Migraine often does this sometimes without a headache especially if someone took a headache medicine like ibuprofen. Methamphetamine or cocaine often causes this even if there is not a serious cardiac condition occurring.
